An erection lasting more than four hours is an emergency department, not a wait-and-see.

How to use a penis pump

Sessions of 10 to 15 minutes, 20 at the outside. Longer is not better and the risk climbs quickly.

Release the vacuum every 3 to 5 minutes and let circulation return before pumping again.

Stop at discomfort, not at pain. By the time it hurts, damage has started.

Never pump to the maximum the device can reach. The ceiling is what the pump can do, not what you should do.
